Find a Provider
FIND A PROVIDER
Your primary care provider (PCP) has privileges at a hospital in the US Family Health Plan provider network and will refer you to specialists and other medical professionals who are also connected with that hospital. We call this group your "referral circle"— it's a system that ensures better communication and coordination of care between your PCP and other providers.

Please Note: The physicians and medical groups in the US Family Health Plan Provider Network are independent practices that contract with—but are not employed by—the Plan. While every effort is made to keep these listings up to date, some providers may have changed their availability or closed their panels to new patients. If you are selecting a new PCP, it's a good idea to call that provider's office first to make sure he or she is accepting new patients.
