USFHP Maxor Mail-Order Pharmacy

Follow these easy directions to get your refills by mail.

 

1. Look at your US Family Health Plan Maxor Pharmacy receipt or prescription bottle. If your prescription shows you have refills remaining on your medication, send your Mail Order Request Form and your appropriate copayment for the prescription to this address:

You may also order refills by calling the 24-hours, toll-free refill order line at 1-866-408-2459.

2. Your copayment can be in the form of a check, money order or credit card. To pay by credit card, fill out the information listed directly on the Mail Order Request Form. Up to six (6) prescription on each order form.

3. In order to get your refill mailed to you before your current medication runs out, please mail your request to arrive at the pharmacy 14 days before your supply ends. Also, allow extra delivery time during holidays due to delays at the post office.

4. To transfer an CVS Pharmacy prescription to the US Family Health Plan Maxor Pharmacy:

• 1. Use the Mail Order Request Form
• 2. Call the toll-free number (1-866-408-2459) and speak to a customer service representative.
• 3. Mail a duplicate of the CVS receipt to:

Important: Make sure you have the CVS phone number, prescription number, drug name, patient's name and patient's phone number available.

5. Requests for up to a 90-day supply of medication are available. They will be honored if your doctor has written a prescription authorizing that amount.

6. Requests with handwriting that is hard to read will take much longer to renew so please write clearly so that you can be better serviced. Also note that some medications cannot be mailed. These include refrigerated medications, insulin (during the hot months) or very fragile products.

7. If your address or phone number should change, list the change on the Mail Order Request Form to insure that your medication is mailed to you at the correct location.
