All medications prescribed by your designated PCP or a referred specialist are covered*. The low co-payment ($3 for generic, $9 for brand-name, $22 nonformulary) eliminates the stress of paying for needed prescriptions.
Prescriptions for US Family Health Plan members in southeast Texas and southwest Louisiana are provided by Maxor Pharmacies. Maxor Pharmacies offer services to patients at retail pharmacies, outpatient pharmacies, partnership pharmacies, and our mail order pharmacy. Maxor's mail order pharmacy offers:
