Who We Are

US Family Health Plan owns and operates three health centers in New York: The Bay Ridge Family Health Center in Brooklyn near Fort Hamilton; The Mitchel Field Family Health Center in Garden City on the Mitchel Field Navy Complex; and, The Fort Wadsworth Family Health Center located on the Fort Wadsworth Coast Guard base in Staten Island.  Our centers proudly provide primary care services to USFHP members. Our offices are staffed with board certified physicians are knowledgeable and friendly clinical and administrative support staff.
